5/1/2005 PacifiCare CARA I Focal Renewal kits have been mailed to Groups and Brokers. PacifiCare no longer accepts old forms, so please only use new ones enclosed with the kit. All changes must reach our office no later than 5/16/2005. No Exceptions!
Individual & Family Plans
Effective 5/1/2005, rates will be increased and four new plans added: (1) PPO 60-50/2500; (2) HDHP 35/80-60/2700; (3) HDHP 100-50/5000; (4) HMO 35/50. For more details, please call our office.
Health Net
Small Group
Effective 5/1/2005, rates will be increased. All PPO plans, except PPO 10 plan, will change its out-of-network benefits. Underwriting Guidelines have been modified. Call our office for details.
Individual & Family Plans
Effective 7/1/2005, first month premium is required to be submitted with Initial Enrollment Application. If it is not remitted, then the application will be held up for approval until it is received.

• Group - Employer Paid plan for companies with 3 or more employees - up to $50,000 guaranteed issue limit.
• Individual - Voluntary term life plans with level premiums to age 100.
