PacifiCare Small Group
Renewal changes for the 5/1 CARA-1 focal renewal have been completed. Please be advised anyone requesting a plan change will receive new ID cards within the next three weeks.
For CARA-2 (new and renewal cases), effective 7/1/05, rates and benefits will be changing. We will have more on the changes in next month’s News Update.
Please note: Effective immediately, SDHP plans are no longer available to CARA-1 and CGA groups, except CARA-2 groups.
PacifiCare Individual & Family Plans
Effective 5/1/05, IFP rates have changes and two new plan designs have been added as HSA-Compatible plans:
1. HDHP 35/80-50/2700
2. HDHP 100-50/1000

Health Net Small Group
RAF Program Revisions effective 5/1/05.
Here is the complete RAF structure.
Groups of:
• 2 to 5 start a 1.10
• 6 to 9 start 0.90, based on employees’ Individual Health Questionnaire
• 10 to 14 start a 0.90, based on employers’ group-level Health Questionnaire
• 15+ is guaranteed at 1.00 OR employers may complete a group-level Health Questionnaire for a 0.90
Rate Reductions, effective 6/1/05, for the following plans:
• EOA 25
• Silver Network plans: HMO 20 & HMO 40
• HMO 40
• All other plan rates remain the same
Health Net Individual & Family Plans
Final Reminder! Effective 7/1/05, first month’s premium is required to be submitted with ALL Enrollment Applications. If it is not remitted, then the application will be held up for approval until it is received.

An Important Notice will be sent to your clients by Health Net.
