• Minimum contribution $100 or 50% of lower cost plan
NEW HMO XTRA Value plans will be launched.

• SignatureValue (HMO) 30-40/70 will also be discontinued and replaced with a new plan, SignatureValue (HMO) 35/600d
• BENEFITS CHANGED for PacifiCareValue (HMO) 10/500d and SignatureOptions (PPO) 70-50/2000 plans
