- Renewal kits have been sent.
- During Open Enrollment, Groups may change plan(s) and Employees may add/remove eligible dependents.
- Effective 5/1/06, SignatureValue (HMO) 30-40/70 plan will be discontinued and replaced by a new plan named SignatureValue (HMO) 35/600d.
- The new SignatureValue Advantage (HMO Value) plans are identical to the traditional SignatureValue (HMO) plans. Enrollees will need to select a Primary Care Physician from Participating Medical Groups in Kern, Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Francisco, and Santa Clara counties within the PacifiCare SignatureValue Advantage Network. Note: SignatureValue Advantage cannot be offered alongside SignatureValue plans.
- All changes are to be received by May 19, 2006. No Exceptions!!! *

Effective 7/1/06, Shield Spectrum PPO Savings Plan 1250 will be withdrawn. Your renewing clients will not be affected until their renewal effective date, at which time they will need to select a new plan.
